Phimosis

Phimosis is a condition where the foreskin of the penis cannot be fully retracted over the glans (head) of the penis. While it is common in infants and young children, it usually resolves on its own by adolescence. In adults, phimosis can be more concerning and may require medical treatment.

Symptoms of Phimosis

Phimosis and its causes are characterized by one or more following symptoms:

  • Inability to retract the foreskin over the glans
  • Pain or discomfort during urination or sexual activity
  • Swelling of the foreskin
  • Recurrent infections of the foreskin and glans (balanitis)
  • Ballooning of the foreskin during urination

Etiologies (Causes) of Phimosis

Phimosis can have various underlying factors, including:

  • Physiological Causes:
    • Natural adhesion of the foreskin to the glans in infants and young children
  • Pathological Causes:
    • Recurrent infections (balanitis or balanoposthitis)
    • Poor hygiene leading to infections and inflammation
    • Scarring due to forced retraction or previous infections
    • Chronic conditions such as diabetes that can predispose to infections
    • Skin conditions such as lichen sclerosus, which can cause hardening and tightening of the foreskin
    • Balanitis xerotica obliterans

Investigations and Diagnosis

Diagnosing phimosis typically involves:

  • Medical History:
    • Identifying symptoms and any recurrent infections or trauma
    • Assessing underlying conditions such as diabetes or skin disorders
  • Physical Examination:
    • Evaluating the foreskin and glans for signs of infection, scarring, or adhesions
    • Checking for other conditions that may be contributing to phimosis
  • Laboratory Tests:
    • Occasionally, swabs or urine tests to check for infections
    • Blood tests to assess for underlying conditions like diabetes

Treatment Options for Phimosis

Effective management of phimosis often depends on the severity of the condition and underlying causes:

  • Conservative Treatments:
    • Topical Steroid Creams: Applying prescribed steroid creams to the foreskin to reduce inflammation and promote retraction
    • Gentle Stretching Exercises: Gradual and gentle stretching of the foreskin to improve elasticity
    • Good Hygiene Practices: Ensuring regular cleaning of the foreskin and glans to prevent infections
  • Medical Treatments:
    • Antibiotics or Antifungal Medications: Prescribed if infections are present
    • Treatment for Underlying Conditions: Managing conditions like diabetes or skin disorders contributing to phimosis
  • Surgical Treatments:
    • Circumcision: Surgical removal of the foreskin, often recommended for severe cases or recurrent infections
    • Preputioplasty: A less invasive surgical procedure that involves making small incisions in the foreskin to loosen it and allow for retraction
