Symptoms of Epididymitis-Orchitis (EO)
Epididymitis-orchitis can present with the following symptoms:
- Scrotal pain and swelling
- Tenderness in the testicles and epididymis
- Redness and warmth of the scrotum
- Painful urination or ejaculation
- Fever and chills
- Discharge from the penis
- Blood in the semen
- Frequent or urgent need to urinate
Etiologies (Causes) of Epididymitis-Orchitis (EO)
Epididymitis-orchitis can have various underlying factors, including:
- Infectious Causes:
- Bacterial Infections: Retrograde ascent of pathogens along the penis is the usual route of infection. It is commonly caused by sexually transmitted infections (STIs) such as Chlamydia and Gonorrhea.
- Viral Infections: Mumps virus can cause orchitis, particularly in post-pubertal males
- Non-Infectious Causes:
- Trauma or Injury: Physical trauma to the scrotum can cause inflammation
- Chemical Irritation: Reflux of urine into the epididymis, often due to heavy lifting or straining
- Autoimmune Conditions: Rarely, autoimmune reactions can target testicular tissue
- Postinfectious: An inflammatory reaction to pathogens in the younger age group
- Certain Medications: e.g. amiodarone
- Risk factors include: Risk factors for epididymitis in all men include sexual activity, strenuous physical activity, bicycle or motorcycle riding, prolonged periods of sitting (e.g., during travel, with a sedentary job), recent urinary tract surgery or instrumentation and anatomic abnormalities
A condition we have to exclude, especially in younger men, is testicular torsion, which can present in a very similar way. Testicular torsion is a medical emergency where the spermatic cord, which supplies blood to the testicle, twists, cutting off the blood flow.
Investigations and Diagnosis
Diagnosing epididymitis-orchitis typically involves:
- Medical History: Identifying symptoms, recent infections, sexual activity, and any history of trauma
- Physical Examination: Evaluating the scrotum for swelling, tenderness, and redness. Checking for any signs of discharge from the penis
- Laboratory Tests:
- Urine Tests: To identify bacterial infections or STIs
- Blood Tests: To check for elevated white blood cell count indicating infection
- Urethral Swab: For STI testing
- Ultrasound: To assess blood flow and rule out testicular torsion
Treatment Options for Epididymitis-Orchitis (EO)
Effective management of epididymitis-orchitis often depends on the underlying cause:
- Antibiotics: For Bacterial Infections; Prescribed based on the specific bacteria identified
- Pain Relief: Over-the-counter pain relievers (e.g., ibuprofen, acetaminophen) to manage pain and inflammation.
- Supportive Care: Bed rest, scrotal elevation, and applying cold packs to reduce swelling and discomfort.
- Treating Underlying Conditions: Managing UTIs or prostatitis that may have contributed to the development of EO.
Follow-Up and Management
After initiating treatment, regular follow-up is important to monitor progress and ensure resolution:
- Monitoring Symptoms: Ensuring pain and swelling are decreasing.
- Assessing Treatment Effectiveness: Checking for infection clearance.
